3 Monitors on 6950

I have an 6950 with
2x dvi
1 HDMI
2 Mini DP
when i connect 1 dvi and 1 HDMI , all works fine, when i add the Mini DP adapter to HDMI  one of the outputs go black

How i have to config the outputs to work on all 3 monitors?

just make sure to use an active Mini DisplayPort to HDMI Adapter like our CAC-1170 (plus a normal HDMI Cable).
