We do not understand well your question but I think you want to know how to set up a system that supports 5 displays simultaneously.
You need a mainboard that supports PCI-E 3.0 with at least one PCI-E 16x slot.
For graphics card you can use one that has at least native 5 or more ports or you can use a combination of a graphics card and a MST Hub.
As we do not know what kind of monitors you will be using, here are some recommendations;
