Mac Pro 2013 Samsung UHD 4K 60hz screen goes on/off Club3d DP 1.2 to HDMI 2.0

I got the Club3d DP 1.2 to HDMI 2.0 adapter and a high speed cable and I cannot get my samsung UN40KU6300 to hold a 60hz 3840×2160 resolution for more than a minute, then it cuts in and out a few times until it goes dark. I have followed all advice, all setting are off, UHD is on, HDMI is plugged into input 1 (DVI) Switchrex is installed and a custom resolution is set at proper setting, and TV is set to monitor 2. Any ideas? Buy a sony or LG and see if they work? Order a Club3d HDMI cable? The firmware is not the newest but support said they did not think that was it. I tried to update on my MacBook air booted into windows but I got errors.

The weirdest part is that the signal locks in and looks good just long enough to make you think it will work.. then it all goes down hill.

I move the computer to an upstairs Visio 4K TV and had the exact same issues when trying to push 60 Hz at full resolution must be a driver software operating system etc. issue
