I had a similar issue—turns out it was the cable not properly supporting 4K@60Hz even though it’s supposed to. Make sure you’re using the latest Intel drivers and check the CAC-1087 firmware via Club 3D’s site. Some users needed a firmware Basketball Stars update for 4K to work properly.
