I have returned my CSV-5400 to Club-3D and have got the firmware updated but sadly it has still not fixed the problem.  I am now back to square one, only 2 displays will work concurrently and 2 HDMI + 1 DVI or 1 HDMI + 2 DVI wont.  Any further suggestions?
