A 7850 RoyalKing runs standard at 910 MHz and according to AMD it can go up to 1 Ghz.
There can very well be differences between different HD 7850 GPU’s with regards to how far you can overclock them. That you cannot reach 1200 Mhz does not surprise me, and i also do not recommend that.
That the card won’t run properly in default settings is weird. Did you clean the card lately (get all dust out).
Please let me know your full configuration.

Cables are fine, MST Hub is fine, Graphics Card is fine, must be the software.
Win10 i asusme. IF you used an update to go to WIN 10, please try this:
Make sure your documents/pictures/music etc. is safe.Ā Go to settings, recovery, reset this PC, chose Remove Everything (you should have your personal files safely copied on USB stick or USB HDD).
Such clean install of WIN10 normally make the system work better and will also remove older versions of the OS which safes you lots of space.

HDMI1.4 does not have enough bandwidth to put 3840×2160 at 60Hz thru.
A resolution of 3840×2160 at 30Hz is max.
You would need a DisplayPort1.2- or a HDMI2.0- or USB3.1 Type C output on the notebook to make that possible.
Apart form that i don’t think Intel Integrated Graphics HD4400 is capable of generating 3840×2160 at 60Hz.
Beautiful notebook (wish i had one) but not to power a 4K screen at 60Hz.

The fact that 1 screen is running off the R9 280 tells me the system is ok and the grahics card as well.Ā
In the Bios of the mainboard you can select the option to have the system initialize on the PCI-E Graphics or the Integrated Graphics. Normally this will be set to “Auto”, i prefer to disable the integrated graphics completely or chose for the option “PCI-E Graphics only” which normally doe sthe same thing. This means the Graphic outputs of the Mainboard dont function anymore and you should use The R9 280.Ā
Please check if the DVI-Output-Connector on the graphics card is ok. In this female connector are little gold plated connection pins that sometimes get damaged when attempting to force in the DVI plug the wrong way (or something like that). Please check if the DVI Cable is OK and last but not least, please check if the Screen or DVI input if the Screen is ok.

What Graphics Card are you using?
To make Eyefinity 6 work in general ;
OS WIN Vista or newer.
Make use of the DisplayPort output on the card (easy on this card, because this card only has DP outputs)
When connecting Screens with DP input you can connect those directly with ānormalā or āpassiveā cables.
When connecting Screens without DP input, so DVI or HDMI, you will need active adapters with Eyefinity support for all screens after the first 2.
So in your case like this:
Screen 1 : Connect with normal/passive adapter cable plus HDMI Cable to the TV
Screen 2 : Connect with normal/passive adapter cable plus HDMI Cable to the TV
Screen 3 : Connect with an active adapter with Eyefinity support plus HDMI Cable to the TV
Screen 4 : Connect with an active adapter with Eyefinity support plus HDMI Cable to the TV
Screen 5 : Connect with an active adapter with Eyefinity support plus HDMI Cable to the TV
Screen 6 : Connect with an active adapter with Eyefinity support plus HDMI Cable to the TV
I do not know the Delock Active Adapter you mention, it seems to be ab active one but i am not sure it actually support Eyefinity, like our CAC-1153 does for sure.

Also please note that there’s a limitation to the length of the HDMI you can use. Normally cables over 3 meters will need to be active.
And last but not least, please check on the configuration of the screens, although you can output 6 x 1920x1080p at 60Hz you cannot place all 6 screens vertically next to each other, please check this link:

I am asuming yourĀ P645-S notebook has a DP output.
You connect the MST Hub directly into the DP Output on the notebook or thru the Docking Station ? To do it directly is best.
Also the MST need power from the USB port and not always all usb ports are able to do that, so please check how that is on your notebook.
