The Quadro NVS 295 is a Tesla GPU, and i am quite sure that this is not compatible with the adapter in the sence that it can output 4K 602Hz over it. With Kepler GPU’s it is already a “challange” but with older GPU’s that would become “close to impossible”. Also are you sure a NVS 295 can output 4K 60 Hz … i think max is 2560×1600 ,,,

I have heard quite a few strange behaviours with regards to R9 series cards in combination with AMD Drivers after 16.8.3. It seems that from 16.12.2 onwards most issues are relatively well under control again.
Please make a new clean driver installation by, certainly when using update feature for updating driver old files can/will remain in the registry of windows and those can be “in the way”. Pleas do this:
I think Driver version 16.8.3 is overall working very well for R9 series. I know AMD made many driver updates after that, however all aimed at RX400 series and non that I know of for R7/9 200/300 Series. I do hear end-users with R7/9 200/300 series Cards reporting that 16.12.2 (or later) also works quite decent for them. Did not test that myself yet.
Please uninstall the current Graphics driver and all related Graphics software like Afterburner/GPU-Z or whatever graphics related software you might have installed. Best way to remove Driver Software is to boot Windows in Safe mode, so the registry can also be cleaned. When you system does not allow you to do that, you can also use a program like DDU to remove such software: http://www.wagnardmobile.com/DDU/
Reboot and manually download and install 16.8.3 graphics driver (don’t let auto installer from Windows do that) You can find here:

i am going at assume you have a laptop (without dual graphics feature) on windows.
Please do this:
Disconnect the docking station.
In install all Displaylink software/drivers.
Reboot system.
Dowload and install latest available Displaylink drivers from here:
http://www.displaylink.com/downloads/windows
Reboot ssytem again and connect Docking Station, second screen should work ok now.

The system will get EDID info from the screen at boot. Screen will tell the system what the screen can handle and system will provide if it can.
When this “digital handshake” does not go well, the system will “make up” something itself.
There shold be a work-around for that or sometimes a firmware update can help. But let’s

What system you have (CPU / Operating System) ?
What Graphics is inside ? (Older than Maxwell) ?
What Screen do you have (brand and type) ?
What HDMI Cable do you have (brand and type and/or 4K 60Hz / 18Gbps capable ) ?

That is very rare, what Screen you have ?
Normally DVI inputs with DualLink support are downwards compatible with SIngle Link (will support up to 1080p over SL where you can go up to 1600p over DL of course).

Did you try making custom resolution for 3840×2160 at 60 Hz with timing Stadard set to CVT-RB (reduced blanking) using a program like SwitchResX ?
Or roll back to El Capitain if that is an option for you
sounds like the adapter loses not only signal but also power when system is put to sleep, and is not woken up when the system does …
Normally jsut reconnecting the adapter will make it rebuild the signal and work properly again. No solution fo this one yet from R&D 🙁
When running in 3840×2160 at 60Hz in 4:4:4 the BPC needs to be at 8Bit BPC otherwise the signal will get too big …

Please also check if your screen can do 4:4:4 at 4K 60 Hz, not all screens can do that …
