i think it is your kepler GPU that is not able to do it.
I mean Kepler based Grpahics is not working (well) with DP-to-HDMI converters at all, where as newer GPU’s do not seem to have that issue …

A Displayport 1.2 output has 21.6 Gbps of Bandwidth and an HDMI 2.0 has 18Gbps of bandwidth. So with the CAC-1070 / CAC-1170 the signal needs to be no bigger then 18Gbps.
3840×2160 at 60 Hz in 4:4:4 8 Bits BPC = 17.82 Gbps
3840×2160 at 60 Hz in 4:4:4 10 Bits BPC = 22.28 Gbps, Will not fit !
3840×2160 at 60 Hz in 4:4:4 12 Bits BPC = 26.73 Gbps, Will not fit !
If your screen is telling the system it can do 3840×2160 in 4:4:4 the AMD driver will most probably automatically switch to 8Bits BPC. It is indeed not prctical that you cannot chose to switch to 4:2:2 or 4:2:0 so you can up the BPC but i cannot help you with changing the AMD Driver :-S
You will probably need a separte program to be able to do that ..

Why do you want a Dual Link DVI output ?
I mean with Single Link DVI you can also do 1920×1080 at 60Hz.
You can do that with CAC-HMD>DFD

You can of course also use a CSV-2300D or CSV-2604 like you already suggested,

The adapter does support 12Bit BPC but does not forward the HDR data.(Support for 10/12bits bpc is not the same as support for HDR.)

I think that the CPU, or better yet, the integrated Graphics of the CPU, does not support Multi Stream Transport (MST). Therefor the video signal goes thru as is, seeing 1 screen but no other signal goes thru.

I am sorry to say that i did not get any feedback form R&D other that what we covered already.
Only thing i am not convinced about is the 16.12.1 driver. I wrote before that i have end-user reporting that 16.12.1 works quite well for them in similar setups, this is not tru, i meant 16.12.2 !
So 16.8.3 or otherwise go for 16.12.2.
Please make that a clean driver setup …
Please uninstall the current Graphics driver and all related Graphics software like Afterburner/GPU-Z or whatever graphics related software you might have installed.
Best way to remove Driver Software is to boot Windows in Safe mode, so the registry can also be cleaned. Especially by using auto-update feature for updating drivers old files remain on the system (in the registry) which can be “in the way”. When you system does not allow you to do that, you can also use a program like DDU to remove such software: http://www.wagnardmobile.com/DDU/
Reboot and manually download and install 16.8.3 graphics driver (don’t let auto installer from Windows do that) You can find here:

When done do the Custom resolution for 3840×2160 at 60Hz with Timing Standard set to CVT-RB.

Yes a MBP early 2015 supports 4K 60Hz output over its thunderbold2 output.
Setups with TV’s normally work best by making a custom resolution for 3840×2160 at 60 Hz with TIming Standard set to CVT_RB (Reduced Blanking). You will need a progrma like SwitchResX to do that timing standard setting.
