The White with Red box is the new packing, where the back verison is the “old”,bu thte product CSV-5200 indeed is the same.
Max Resolution for 1 screen = 3840 x 2160p at 60Hz
Max Resolution for 2 screens = 2560 x1600 at 60Hz
A CSV-5200 can do stream cloning. meaning it can copy a single video signal 4K60Hz to 2 different outputs, but it will be the same signal (mirror) only.

worked very well for me where i also had sounds issues with newer drivers.

And i think you are correct, this CAC-1507 is not an Active Adapter, it does support 4lanes DP 1.2, but does not convert anything, so no need to be active.
In general with regards to adapters, they are called passive when they just change the gender or connector but no signal conversion tkaes place. When signal conversian does take place the adapter will need to have a receiver and transmitter chip inside which would make it called “active”.

We do have newer firmware, however i am not convinced this will solve your issue, i will have that send it over by email from [email protected].
With TV’s it normally works best to make a custom resolution for 3840x2160p at 60 Hz with Timing Standard set to CVT-Redeced Blanking (RB). You will have to make that in the Intel Control panel advanced settings first for the correct screen, before you will be bale to select that option. Sometimes also requires a reboot.
What Graphics Driver version are you using ?

A USB type C output does not necessarily supports output for video signals. The output will need to have Display Alt Mode support for that to work. Your MacBook Pro has Thundelbolt3 which has full support for Displayport, so that is not the the problem.

Yes it does, however limited to the bandwidth of dp1.1. 1080p at 60Hz takes Aboutaleb 3.73 Gbps , so more toen 2x 1080 p at 60Hz wont fit

Are you sure that your Notebook ( i assume) has support for DP1.2 output ?
If it is DP1.1 that would explain why you don’t get more than 2 x 1080p over it …
Also many Notebooks have limitation with regards to the amount of external screens you can connect. Often not more than 2 external screens is supported so 3 in total including the Notebook screen.

Omdat de schermen 1920x1080p 60Hz native zijn, maakt het niet zoveel uit want alle uitgangen van de VGA Kaart kunnen dat doen. Ik zou wel aanraden om beide schermen op een zelfde manier aan te sluiten ten einde zo min mogelijk verschil tussen beide te hebben. Als je al DVI Kabels hebt kan je die gewoon gebruiken zonder bang te hoeven zijn dat dit minder kwaliteit zou opleveren, als die kabels goed zijn is dat zeker niet het geval.
