If you used the upgrade feature to install latest driver i advise you the following:
Un-install all VGA related software, including Crimson, driver and other software like Afterburner or other tuning software. It is important to do this on safe mode so that the registry can be cleaned. If the system does not allow you to do remove In safe mode, please use a program like DDU: http://www.wagnardmobile.com/DDU/ . Reboot system (normal mode). Windows will ask to install driver, just click that away (close). Go to AMD for download and installation of latest driver for your card and OS. Please do not let any (windows)  auto-installer do that.

Checklist
* Make sure the HDMI cable you are using can do HDMI2.0 or 18Gbps.
I know it sounds a bit âeasyâ but many cables do state to support 4K but actually do not support that in 60Hz and/or the full bandwidth, that certainly goes for cables longer than 3 meters. * Make sure the HDMI input used on the TV is capable of doing 4K at 60Hz often there are more HDMI inputs, but not always all are HDMI2.0 spec.
* On many TVâs you need to turn on “UHD Color” or âAdvanced or Enhancedâ setting manually in the settings of your TV for the HDMI port to which your pc is connected.
* Also it could help when connecting 2 screens, to set the screen connected thru the Adapter as second/secondary screen
* I advise you to make a custom resolution, set resolution to 3840×2160 at 60Hz and set Timing standard to CVT-RB (reduced blanking)
* Please make sure you do not have any Displayport Splitter or Hub attached to the Graphics Card.
*Please note that in some cases configurations in SLI and/or Cross-fire might casue issues. Please disable SLI and/or Crossfire if so.
* Color Depth need to be set to 8 bpc (when running 4:4:4).
What are the 4K formats supported by the adapter ?

You did a lot of testing already.
With Intel HD4600 graphics this should work:
Manually download and install (donât let that auto-updater from Intel do that) Intel Graphics Driver pack 15.40.25.4463, which contains DisplayAudio Driver 6.16.00.3197 plus Graphics driver 20.19.15.4463.
Open Intel Graphics Control Panel, Chose âCustom Resolutionsâ (left bottom option)
It will asks you if you wish to continue; YES
Standard the âBasicâ setting is shown, click on âAdvancedâ , Fill in; Width 3840 ; Height 2160 ; Refresh Rate 60 ; Timing Standard CVT-RB ; and click on âADDâon the right bottom of the window.Â
Now this resolution I added to the system and can be chosen.
Go back to âGeneral Settingsâ(left top) , it will show the basic options and here you can now choose the 3840×2160 resolution and 60Hz with CVT-RB.

To be able to give you decent advise i will need more info;
brand / Type of your screens
What system you use (OS) and what Graphcis is inside ?
What cables are you using / how are the screens connected ?
In general, a DiaplsyPort 1.2 Output has enouguh bandwidth to send 4 x 1920×180 at 60Hz thru. When the system supports MST Feature (Windows 7 does support that) you are able to send 4 inividual video signals thru a single DP1.2 output and the MST will distribute those to the Screens. No need for drivers or anything like that. It could be that the in the Graphics Settigns of the system you will nee to enable the screens, but the sytem should recognize them when connected.

There is no real trick to it. If the Source system support MST Feature this should work and under WIN 10 the MST Feature is supported so that is not it. On a SP4 it is tested and Microsoft has also certified the CSV-5200 / CSV-5300A / CSV-5300H / CSV-5400 / CSV-3203 for use with their SP 2/3/4.
Please Disable all Energy Safe Modes your system and or screen might support.
Use passive cables to conenct your screens to the HUB since the HUB is already an Active device.
Please check for firmware update for the screens.

Supported resoltuions are more dependent on the Bandwidth of your DP Ouput and system graphics, then on the MST Hub. I mean if the signal fits thru the DP Ouput the MST Hub won’t stop it …
